If you are missing certain usage type sizes on download for certain images then it's probably the case that the 'Omit if lower resolution' option has been ticked for the usage type size in question and the image is a low resolution one.

To check and correct this problem, login as an admin user and go to:

Admin > Download Options > [edit sizes] 

 and look at the details for the usage type size in question.
